Ord has graciously allowed us to post their recertification application to show one community’s response with the process. The budget had deliberately been excluded for confidentiality.
Reapplication is due 3 months prior to 5th annual date of certification. For example, if your certification date was July of 2010, then your reapplication would be due to department April of 2015.
